We are here to help you navigate a PATH through your medical journey.
Patients and Their Families
Patients have run the gamut from chronic to terminally ill, from the elderly to children with special needs. In many cases the patients cannot advocate for themselves because of their illness. It is critical that ahead of time they select, train with, and develop plans with someone to fill the patient advocate role. We often teach a family member how to take on this role and advocate for their loved one. Other times, it may be a close friend who fills this role. In fact, teaching them both together prepares the best team.
Without this critical role being filled, gaps in care occur and mistakes can be made. No matter how dedicated, no medical practitioner can devote 24/7 attention to any one case. This is the uniqueness of patient advocacy done by the patient or a family member. |

Our Assistance and Training
We teach advocacy and problem-solving skills, helping patients and families evaluate their own critical and special needs.
- Seminars - learn to put yourself on your medical team, what information doctors seek that you can provide, how to build coalitions through CARE conferences and more. Cost $50.00 per patient team of two.
- One-to-one assistance - we are there during any medical challenge or crisis to help you prioritize and problem- solve your unique situation. Cost: We work on a sliding scale depending on your need and frequency of contact.
Types of Assistance & Training Provided
- create a PHR (personal health record) to improve communication between the patient and the medical team
- help each family team (patient & family caregiver) develop a strategic plan for handling patient's needs and medical challenges
- help build a support team to meet patient needs
- research and problem solve in areas of treatment options, facilities, physicians, and medical coverage issues
- teach the patient and/or family member how to research, network and obtain knowledge about their disease and treatment options
- find or start a patient/family support group
- find local sources for loaned medical equipment
